Does anyone have any evaluation papers they have made up? I want to do one for each of my dogs, but if someone already had a layout they didn't mind sharing, that would be AWESOME

I like the one from the Akita's site. That's a nice, efficient format.
As an add-on to this, Pat Taylor's Puppy Puzzle says that for a show potential, none of the puppy's traits should be less than a "3". Everything on that puppy should be average or above. Anything else should go to a companion home. I personally think that's a bit harsh, but I wouldn't keep a pup with more than one trait under a "3", I don't think. And average means an average example of that breed that you would see in the show ring, so that's a high standard.
